Incorrect 1099-R

I have new pension administrator and they are insisting that a 12/24/24 payment was part of 1099-R for 2025. When investigating I found I actually received 13 payments in 2024 and 11 in 2025 due to them holding late Dec payment until Jan 2 this year. They will not send me a corrected form. I just amended 2024 to show 13 payments in that year. How do I do 11 payments in 2025 in freetaxusa and not the 12 they have on 1099-R. They are refusing to fix.

Hello Papaduke,

The IRS does suggest if the issuing company will not send a corrected 1099-R to contact the IRS and they will contact the company.

You may need to file form 4852(Substitute Form for 1099-R), print it out and include it with your 2025 return. I will include the IRS guidance for this below:
