Lost W-2g form, but I know most of the numbers

In 2024, I received a W-2g form from the Greektown casino in Detroit for a $1500+ slot machine winning. I have the total amount, federal and state withheld, date, etc., but unfortunately I do not have the federal or state identification numbers for the casino.

I have requested a replacement W-2g form from the casino, but it has not yet arrived.

I know that the IRS will kick out the return if it doesn't have the correct identification numbers.

Is there any way that I can enter the amount of the winnings and the state and federal withholdings on my taxes without having the identification numbers?

    Hi TGD,

    It is best to wait for the Form W2-G since you know it's coming.

    An alternative that may or may not work if you don't want to wait for/or have your W-2G from the Casino, is to report your winnings as "Other Income." Please be aware it may be rejected without correct ID numbers as you mentioned. Alternatively, you may get a letter asking for a copy of your W-2G.

    Follow this menu path: Income > Uncommon Income > Other Income > Did you have any of this other income?
