1099-R
Today is 5 March 2026. I want to go on record at this time to report that the FreeTaxUSA software did not include two IRS Form 1099-R statements in my 2025 federal income tax return.
Both of the missing IRS Form 1099-R statements were input online by me and appeared in the supplemental Income Summary Statements provided by FreeTaxUSA.com. My 2025 federal income tax return was filed on 4 March 2026.
Both of the original Form 1099-R statements were marked in Box 2b as "Taxable amount not determined" by the payer. However, Box 2a indicated a taxable amount and Box 7 indicated a normal taxable distribution. The total amount of the two missing IRS Form 1099-R statements is $19,198.
This FreeTaxUSA software error has caused misreporting on my 2025 federal income tax return, understated taxable income on Line 5 of the Form 1040, and required filing a 1040X amendment to the original 2025 federal income tax return.

Did you contact customer support while signed into your account on FreeTaxUSA? What you have described, Box 2a with a taxable amount and box 7 with code 7, will be reported as taxable. How did you determine those amounts were not reported on your tax return?
Was this an IRA/SEP/SIMPLE (Line 4a and 4b) or a pension (Line 5a and 5b)?
I hope you were able to resolve the issue before you transmitted your return so you can avoid an amendment.
