Did not mention I had filed extension
My wife and I filled an extension in May before the deadline and paid the money. At that time, we wanted to file as "married filing jointly". However, on October, we filed as "married filing separately". Unfortunately, on October, I forgot to mention in my Form 1040, Line 32 that we have paid money with the extension filing. I also forgot to upload the extension document. What should I do now? Should I fill out a tax amend?

You may want to file the amendment. The IRS doesn't always apply payments made if they were not claimed on the tax return. If you had to make an additional payment with your original return because you didn't claim the payment you made with your extension as well, that could mean that you should get a refund of some of your tax payments.
In our software, you can enter your extension payment by going to Misc → Extension and Other Payment once you have started an amendment.
Also, if you owed taxes, showing your extension should help prevent you from receiving the Failure to File Penalty.
