FMLA

My husband received FMLA and they only took out taxes for two checks and he called them 3 times to say please take out federal and state tax.I am stumped in doing our taxes this year because of this error.Is there a form to fill out for income tax purpose for the other 10 checks,I know what the amount is for federal and state.

The Family and Medical Leave Act is primarily for unpaid leave, but there are other options to get paid will using FML. You can use: sick time, vacation time, personal time, or paid time off (PTO).
The Form W-4 is used to calculate withholdings for federal. I am not aware of a special withholding form for the other "10 checks" for FML pay. The income is generally reported on your W-2 for state and federal purposes.
If that happens again and his employer does not do withholding, you can make estimated payments online at the IRS.
