1099-R, Box 15: "State/Payer's state no." Mine? Job? Or TIAA in NC?

Hi. On form 1099-R, Box 15, it asks for "State/Payer's state no." What State should go there?

The "Payer" is TIAA, which is located in NC.

My job was in NY. (I will file as a non-resident.)

My residence is in FL.

The 1099R was completed by TIAA and they put "FL" in Box 15. Is that a mistake?

BTW: Box 16 "State distribution" has an amount of $361. (This is the difference between my contribution to the Roth 403b and the Gross distribution.) Which State distributed the $361, and is that state what should go in Box 15?

    The state you're paying taxes to goes in Box 15. But you don't need to enter anything unless you're going to be filing more than one state return and if there was no state income tax withheld.

    Florida doesn't have an income tax, so nothing's going to happen to it if you input it. But I'd just leave the whole thing blank.
