FLI = Local income tax?

Hello all, I am entering my W2 and I live and work in Colorado. I was asked, "Do you have any Family Leave Insurance (FLI) shown in Box 14 or Box 19 of this W-2?"

There is nothing in Box 14 on my W2.

In Box 19, with the label "Local income tax", there is $34.50

In Colorado, we have a program called FAMLI for paid family leave which I know my employer participates in (but I have not used). So do I need to say yes I have FLI and enter $34.50 where prompted? Or because this is labeled "local income tax" is this something else that does not pertain to "FLI"?

    Hello sarahma78,

    The $34.50 in Box 19 of your W-2 may be the Family Leave Insurance. What does it say in Box 20 for the locality name? That may give guidance on what the Box 19 amount is for.

    I would recommend reaching out to your employer to verify what the amount in Box 19 is reporting.

    Hi Sarahma78,

    DnvrCityOT-EE refers to the Denver City Occupational Tax - Employee portion (often abbreviated as OPT or "head tax"). This is a local tax and may be deductible if you use Itemized Deductions.

    Thanks Matthew, but is it Family Leave Insurance?

    My spouse has a clearly written FAMLI (our colorado family leave plan) in box 14 on hers. Different employer, but I feel like if mine intended this that's how they would have labeled it?

    As your W-2 does not specifically list FAMLI or FLI, but instead lists DnvrCityOT-EE I would agree with MatthewD. This is a local tax, which is different from family leave insurance.

    If you believe that your employer withheld family leave insurance, I would suggest that you contact your employer’s payroll or HR department to confirm.
