Issues with Oregon tax form

When I download my Oregon state return, I'm seeing some issues.

A. On Schedule OR-A, lines 2-4 are empty and not populated

B. On line 9, the number is not the sum of lines 5 through 8. Instead, it is 10,000 which I believe is the limit for tax year 2024.

How can this be resolved?
