I moved from Maryland to W Virginia Feb 7th this year.
Do I have to file a Maryland state tax form for 37 days living in Maryland?

You'll file a Maryland return if you meet MD filing requirements for the year.
Per the MD website, you need to file a Maryland tax return if:
- You are or were a Maryland resident;
- You are required to file a federal income tax return; and
- Your Maryland gross income equals or exceeds the level listed for your filing status. The filing levels also apply to nonresident taxpayers who are required to file a Maryland return. Generally speaking, your amount of 'gross income' for Maryland will be the amount of income you received from all sources while you were a resident of the state, plus any income you received from sources inside MD while you lived outside the state.

If I again have FreeTaxUSA do my taxes will you fill out MD Form 109-1 change of address for me or do I have to do it myself? So I have to fill out two state tax forms, one for MD and one for WV?

You will have to fill out the MD Form 109-I. I have included a link below to that form.
In regard to filing two state returns, if you find that you need to file a MD state return, then you will need to file two state tax returns.
